Feel free to copy this image if you want it. I beleive it has everything on it that needs to be there. You can buy sheets of adhesive label material at your local office supply store, or just print it on paper and use clear packing tape over it. To save the image from a windows platform, just right click and choose "save picture as" from the menu.
